Financial Mentoring Agreement and Privacy Consent

1. Financial Mentoring Agreement

I/We have voluntarily requested support from Mangere Budgeting Services Trust (MBST) and have disclosed my/our financial situation for the purpose of receiving budgeting and financial mentoring assistance.

I/We agree to the following conditions:

Authority & Communication

I/We give permission for MBST Financial Mentor to discuss my/our financial affairs, where necessary, with my/our creditors and any other person or organisation acting on my/our behalf.

Financial Commitments

I/We agree that no further debt or credit will be obtained unless discussed with MBST Financial Mentor

I/We will disclose all income and all expenses/outgoings to MBST Financial Mentor

I/We understand that repayment arrangements with creditors will be determined in partnership with MBST Financial Mentor

Engagement & Cooperation

I/We agree to cooperate fully with MBST Financial Mentor, including attending scheduled appointments and maintaining agreed arrangements.

Withdrawal from Service

I/We understand that I/we may withdraw from the service at any time. If I/we choose to withdraw, I/we will inform creditors and any relevant parties directly.

Limitation of Liability

I/We acknowledge that MBST Financial Mentors are not liable for any financial decisions made based on advice, support, or services provided.

Service Withdrawal

I/We understand that if the above conditions are not adhered to, MBST may withdraw services and notify relevant creditors accordingly.

Risk & Safety

I/We understand that if MBST Financial Mentor representative believes there is risk of harm to myself/ourselves or any child/children, MBST may report concerns to the appropriate authorities, including Oranga Tamariki.

Respect & Conduct

MBST maintains a zero-tolerance policy for verbal or physical abuse toward staff. If abuse occurs, services will be withdrawn without exception.

2. Privacy Consent – Privacy Act 2020

I/We confirm that:

  • I/We authorise MBST Management and Staff to represent me/us in financial matters.

  • I/We authorise MBST to collect, use, and disclose relevant personal information for the purpose of providing financial mentoring services.

  • MBST may communicate with creditors, service providers, government agencies, and other relevant organisations as necessary to support my/our financial situation.

  • MBST may negotiate with authorised parties in my/our best interests.

  • I/We understand that MBST funders may review client files for auditing and reporting purposes. Any information shared externally will be non-identifying unless legally required.

  • I/We understand that arrangements made on my/our behalf are made in good faith and that I/we remain responsible for meeting agreed payment commitments.

  • This authority remains in force unless I/we revoke it in writing.

3. Digital Acknowledgement
